VACANCY: Community Support Coordinator (Hospital Discharge)

tsiMORAY, ACVO and AVA, (Grampian’s three Third Sector Interfaces) are each seeking to

appoint a Community Support Coordinator to deliver an exciting project that will be the first of its kind for Grampian.


This two-year project, which has been developed in partnership with NHS Grampian

Endowment Fund and funded by NHS Charities Together aims to deliver a community based model for accelerated discharge of patients to their own home setting. It will create a three-way link between NHS Grampian’s acute services, the three Health & Social Care Partnerships, and a number of community-based organisations based in Aberdeen, Aberdeenshire, and Moray.


One Community Support Coordinator will be employed by each of the TSIs and will be

responsible for activities within the boundaries of their relevant local authority area

(Aberdeen City, Aberdeenshire, or Moray). The postholders will be responsible for

enabling a partnership approach that delivers the following project outcomes:


  • Better integration between the public and third sectors

  • Deeper understanding of the value of the Third Sector in supporting people to live more independent lives

  • Co-produced outcomes meaning better quality of life for people

  • More efficient deployment of NHSG resources (human, physical and financial)

  • Better integrated communities that are empowered to make positive differences to local lives

The successful candidates will ideally:

  • Appreciate the vital role that community-based organisations have in improving people's lives

  • Demonstrate a dynamic and strengths-based approach to partnership working

  • Be people orientated in a way that will enable rapid development of connections and partnerships throughout the lifespan of the project

  • Have working knowledge of statutory health and social care services


Hours:  28 hours per week (0.8fte)

Salary:  £28,693 (£22,954 pro-rata’d)

Duration: Two years on a fixed term basis
